What is a deep plane facelift?

Deep plane facelift works on the skin while mine works underneath the skin. Most facelifts tighten the surface of the skin which causes the effects of tightening the layer underneath to loosen over time as gravity continues to pull down.

Deep plane facelifts on the other hand move that layer directly with the SMAS, the muscles and tissues that hold your face up. So the effect will last since it was never just about tightening the skin.

The term “rhytidectomy” refers to this procedure.

How is a deep plane facelift performed?

  • The Incision: An incision is made below your ear and followed with the fold of skin from your ear to the middle of your cheek.

  • Locating the SMAS: I look for the submuscular aponeurotic system (SMAS) which is the fascia under your facial muscles as well as the fatty deposits you’ve lost to gravity over time.

  • Repositioning: Next I elevate and pull up this entire complex in unison; I don’t do one area first then another.

  • The result: This action lifts and tightens the midface, the neck and the jowl simultaneously.

There are some people who wish to address issues beyond those handled by a standard facelift.

  • During surgery, fat grafting can be performed.
  • Alternatively, fat grafting can also be done in conjunction with an additional procedure such as a necklift, cheeklift or eyelid surgery based upon how you feel your facial structure has altered.
  • Additionally, CO2 laser resurfacing for improving skin texture can be performed concurrently as well.

Signs you may be a candidate for deep plane facelift

The majority of patients I see are typically in their fifties and sixties. However, it is truly a function of how much your face has aged rather than your age. When we begin discussing a potential surgical option, we discuss jowls that form along the jaw line, loss of fullness in the cheeks due to volume settling lower toward the mouth or deeper lines from the nose to the corner of the mouth.

 

I’m looking into assessing overall health status and realistic expectations. As long as you wish to appear rested rather than drastically different we likely will be a good fit. Conversely, I will be able to inform you in our consultation if I do not believe that is what you are seeking.

✦ How will I recover after a deep plane facelift?

Recovery after deep plane facelift

  • In terms of immediate post op recovery, swelling and bruising generally peak and continue for several days. They can be painful but are completely anticipated.
  • I will see you one week following surgery to evaluate both areas treated and remove any sutures necessary at that time.
  • Generally speaking, most of the easily noticeable swelling resolves itself within 2-3 weeks. There can be lingering tension in the lower portion of the face a little longer.
  • Showering stays away for 48 hours.
  • I would plan on 2 weeks away from work and I would not recommend rushing that.
  • Healing at its own pace is what gives you a result that lasts.
✦ Honest risks of a deep plane facelift

Risks of deep plane facelift, honestly

Any type of surgery comes with some level of risk. Rather than you finding information elsewhere, I believe it would be better for me to provide an open and honest explanation of what you may experience post surgery. The bruising and swelling will subside as previously explained however, there are other potential risks that should be named openly:

Nerve injury

Rare, but possible. Numbness or weakness usually settles within a couple of months. In some cases it can last longer.

Hairline shift

Your hairline can shift slightly at the incision. I plan around this but I can't always avoid it completely.

Scarring

Fades over time but never fully disappears. It sits in the natural crease in front of and behind your ear so it's rarely noticed.

Haematoma

A collection of blood can form under the skin in the first week. If it does, it may need draining in clinic.

Asymmetry

Every face has some asymmetry to begin with, so a slight difference between the two sides after surgery is possible.

I will discuss all of these potential risks at length during our consultation, including those aspects which may appear positive.

FAQs on deep plane facelift in Harrogate

Yes. Rhytidectomy is just the medical term for facelift surgery. Deep plane describes which layer I work in during that surgery, the muscle and tissue underneath your skin rather than the skin itself.

£16,000 under general anaesthetic. That covers your consultation through to your final follow-up, nothing added later. If you're combining it with a neck lift and eyelid surgery, the package price is £20,000.

The surgery itself runs 2 to 4 hours, though I'll confirm the exact figure with you at consultation based on what you need done. Recovery is a separate question, most patients take around 2 weeks off work.

I reposition the SMAS, the layer of muscle and connective tissue under your skin, along with the fat pads that have shifted lower on your face over time. That's what corrects the midface, jowls and neck together rather than one area at a time. It's the part a standard facelift never touches.

Often, yes. Fat grafting, a neck lift, cheek lift or eyelid surgery can all be done in the same operation depending on what your face actually needs. I'll be honest with you in consultation about whether combining makes sense or whether it's better done separately.

Three things happen and they happen together. You lose bone density, so the frame that used to support your face doesn't hold you up the way it once did. You lose fat volume too, particularly through the cheeks and temples, so where your face used to be full, it starts running thin. And your skin loses elasticity on top of both of those changes, so what's left doesn't sit as tightly as it did. Most people notice the results before they clock the causes, deeper grooves round the mouth and forehead, volume that's shifted down toward the jawline instead of sitting up near the cheeks.

Both have their place and I'd never tell you surgery is the only option. Fillers can restore some volume and thread lifts can offer a temporary lift. But neither corrects the layer underneath your skin the way a deep plane facelift does and thread lifts in particular tend to show real effect for months rather than years. If you're after a shorter term refresh, they're worth trying first. If you've already tried them and you want something that actually holds, that's usually when this conversation starts.
